Service Description

This visit will include a review of your current health concerns, recent lab results, past medical history and family history to get the most holistic view of your health. We will incorporate physical exam and additional lab testing as appropriate. Accepting Premera, Lifewise, First Choice and Group Health Options Plans. Discounts are offered for cash payments made at the time of service for patients with other insurance plans. A superbill will be provided for you to submit for reimbursement.
